LCSW - Quality Assurance Clinician

Lamp Community ends homelessness of Los Angeles' most vulnerable individuals, primarily adults with mental illness, through a continuum of services and housing, enabling them to reach their highest level of self-sufficiency and community integration.

The Quality Assurance Clinician will provide review and oversight of all Medi-Cal documentation, client records, and compliance requirements to ensure that Lamp’s Wellness Center operations meet or exceed required documentation and compliance standards for the Los Angeles County Department of Mental Health. The Clinician will also provide staff consultation and trainings as applicable to meeting documentation, compliance standards, and quality assurance best practices.

ESSENTIAL DUTIES & RESPONSIBILITIES
Provide detailed and accurate review of all DMH Medi-Cal documentation including weekly progress notes, Adult Initial
Assessments, Annual Assessment Updates, Client Care Coordination Plans, Discharge Summaries, Progress Notes, and all other clinical and administrative forms associated with each client record; co-sign documentation and collect signatures for documentation as needed
Coordinate with Wellness Center Associate Director to ensure compliance with documentation standards and completion of required documentation
Provide Medi-Cal documentation training to all Wellness Center staff
Audit Wellness Center charts to ensure presence of required documentation, and provide clinical review
Coordinate with Director of Service Delivery and Chief Operating Officer regarding quality and compliance issues
Liaison with Los Angeles County Auditor Controller during Medi-Cal audits
Complete Department of Mental Health Adult Initial Assessments, Annual Assessment Updates, Discharge Summaries, Client Care and Coordination Plans, and Progress Notes as needed

QUALIFICATIONS LCSW
Two years full-time, post-graduate experience working with Medi-Cal/DMH documentation, including both direct practice and review
Highly detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
Strong training, project management, and team-building skills
Able to behave with sensitivity, tact, and responsiveness
Outstanding clinical review skills
Proficient in Microsoft Word, Outlook, Excel, and database applications
Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
Contract and/or quality assurance experience preferred

Outreach Case Manager – HIV/AIDS (Skid Row)

The Advocate – Outreach Harm Reduction Specialist is a community liaison charged with increasing community awareness and access to HIV/AIDS and HCV testing and care, through harm reduction education, HIV/AIDS prevention and/or disease management, and linkage to primary care. Through street outreach and community events, the Advocate will engage homeless adults to determine their HIV/AIDS risk and/or status and triage their need for additional services including emergency and/or permanent housing.

ESSENTIAL DUTIES & RESPONSIBILITIES
Provide targeted case management; coordinate linkage to primary health care, mental health and/or recovery support services for a caseload of up to 24 members diagnosed with HIV/AIDS or at risk of contraction
Coordinate HIV/HCV testing events
Conduct HIV/AIDS groups focusing on education, harm reduction, prevention, and disease management
Provide street outreach to homeless individuals with the objective of engaging them in HIV/AIDS testing, support, harm reduction behaviors, and linkage to primary care
Develop effective, trusting relationship with members utilizing motivational interviewing to encourage engagement, with a focus on facilitating independence and procuring needed services and housing
Respond to community referrals from sources including, but not limited to, 211, BID, and LAPD
Assist in the recruitment, screening, assessment, and enrollment of members into the HOPWA program, including orientation to program policies, community resources, and goals
Collaborate with each member to develop an individual service plan and review and update the plan quarterly and upon completion of goals
Maintain a current, thorough knowledge of community resources and utilize them to provide comprehensive, wrap-around services to members
Work cooperatively and cohesively with other members of the HOPWA and emergency and transitional housing teams, including participation in staff meetings and trainings
Maintain up-to-date, accurate, and complete data, progress notes, and files that include all information required according to agency and regulatory standards
Ensure that all member paperwork and data collection is complete and up to date in the agency database Produce monthly and quarterly reports (e.g., HOPWA, HMIS)
Complete all documentation of services evidencing a minimum of 65% of work hours in direct member service provision

QUALIFICATIONS
High School diploma or GED; Associates or Bachelors degree preferred
Minimum of four years’ experience working with homeless or formerly homeless individuals, preferably those living with mental illness, substance addictions, or HIV/AIDS
Current, valid California Driver’s License with an acceptable driving record
Able to work with minimum supervision
Detail oriented with excellent time management, organizational, written, verbal, and computer skills
Able to obtain and maintain CPR/1st Aid certification
Minimum 30 WPM typing speed preferred
Bilingual in English/Spanish preferred

Line Cook (Skid Row)

Lamp Community ends homelessness of Los Angeles' most vulnerable individuals, primarily adults with mental illness, through a continuum of services and housing, enabling them to reach their highest level of self-sufficiency and community integration. Part-time. Two positions, each 32 hours/week. One is Sunday, Monday, Tuesday Wednesday from 6:30AM to 3PM. The other is Wednesday, Thursday, Friday, Saturday from 6:30AM to 3PM. The Line Cook performs a variety of tasks and operations to prepare and cook a large volume and variety of food items for meal service.

ESSENTIAL DUTIES & RESPONSIBILITIES:
Prepare the main entrée and side dishes for each meal
Follow standard practices, procedures, regulatory requirements, and food preparation and handling techniques
Comply with established standards of sanitation, personal hygiene, and health
Perform clean-up duties to maintain sanitary conditions in the workplace
Serve meals to program participants
Maintain cleanliness of kitchen equipment and food storage and refrigeration areas
Maintain the kitchen shelves, ceiling, floors, walls, and equipment in a clean, orderly and healthy condition
Aid in ordering/purchasing of food, household and maintenance items
Ensure shelf rotation and current inventory of stock items
Aid in minimizing spoilage and waste
Aid in executing Meals and Menus for programs
May be needed for delivering and/or picking up of equipment, supplies and prepared foods

QUALIFICATIONS:
Must have and be able to maintain a Serv-Safe certification
One year’s experience working in a large kitchen
Able to read, speak, and understand directions in English
Able to take direction and execute meal plans
Basic level of written and spoken communication skills
Current, valid California Driver’s License with an acceptable driving record preferred
Bilingual in English/Spanish preferred

Coordinator of Volunteer and Donor Relations

The Coordinator of Volunteer and Donor Relations will be responsible for managing an agency-wide volunteer program comprised of community members and agency clients. These duties will also be coordinated with fundraising efforts, agency-wide communication efforts, and event planning. The Coordinator will be the first point of contact for donor and volunteer inquiries. The Coordinator will liaise with the Finance Department for gift reconciliation and tracking.

ESSENTIAL DUTIES & RESPONSIBILITIES

Develop and manage community-based and corporate volunteer programs
Support agency client volunteers
Manage an agency-wide volunteer activities calendar
Submit and process volunteer applications and respond to all volunteer inquiries in a timely manner
Manage and lead volunteer tours, community presentations, and orientations
Maintain records of volunteer hours, contact information, photo releases, applications, and other critical data
Plan and execute volunteer recognition events
Work with Director of External Relations to communicate volunteer activities, including photos and interviews
Ensure data quality in the Development database, Donor Perfect
Make recommendations regarding data organization and integrity, and reporting tools
Write and produce gift acknowledgment letters, including in-kind and tribute gifts
Produce weekly gift reports, including online giving
Liaise with Finance staff to ensure seamless data transfer and gift reconciliation
Manage all donor stewardship activities
Work with volunteers to maintain impeccable donor files, including foundation, corporate, and individual giving paperwork, including award letters, grant agreements, acknowledgment letters, and proposals
In conjunction with the Development team, execute fundraising events including the annual Fall Affair

QUALIFICATIONS
Bachelors degree
Two years’ experience working with fundraising activities, volunteers, or special events
Must be proficient in Microsoft Office
Able to effectively communicate the importance of Lamp Community’s work in homelessness and poverty, mental health, and harm reduction
Detail oriented with excellent time management, organizational, written, verbal, interpersonal, and computer skills
Able to learn quickly and work effectively with a wide range of constituencies and with minimal supervision
Current, valid California Driver’s License with an acceptable driving record
Strong interpersonal and team building skills
Not-for-profit experience preferred
Experience with donor software preferred
